Dropped Memberships Trend Analysis

I'm looking to do a dropped membership analysis to find out at what point during their membership most people drop (assuming there's a trend), at 6 weeks, 2 months, 3 moths, etc.
I haven't found a report that will provide drop/deactivation dates for all members over time. Or even better if you guys could auto-generate this info in one of your neat graphs for us!

    We would like a way to track the reason behind why a member leaves us. We are currently making manual notes on the member's profile, but there is no way to run a report on this. So, what I would like to do is have an
    automatic screen pop up when canceling a membership that asks for a
    reason, just like it does for holds.
